I am having real trouble using the TOC. When I create a nested TOC, it gets all messed up.
For example, let's say I want the TOC to look something like this:
+1. General Info
1.1 International
1.2 Local
+2. Issues
+2.1 Resolution
2.1.1 Speediness
2.1.2 Overcoming
2.2 Raising
+3. Equipment
3.1 Maintenance
3. 2 Reporting faults
What I am actually getting is something like this (and I've tried resetting, rebuilding, etc):
+1. General Info
1.1 International
1.2 Local
+2. Issues
+2.1 Resolution
2.1.1 Speediness
2.1.2 Overcoming
2.2 Raising
+3. Equipment
3.1 Maintenance
3. 2 Reporting faults
It's a real mess, and a real pain.
Can you help me?

You mean after the CHM compiling? It's a problem of the HTML Help workshop. I'm unsure but I remember that if a chapter has a subchapter, HHWS doesn't like any topic to be inserted in the chapter (just subchapters). Or something like.

Yes, it's after I compile to CHM.
In PaperKiller, the TOC looks just fine, but when I compile, the resulting help file has, for example, just one major Topic (+1. General) and all other topics (even other Major Topics eg. +2. Conditions) come under just that one Topic.
Is there no workaround. Such a limitation destroys the usefulness of PaperKiller. Surely it's possible to have a properly TOC formatted help file? It seems to work on other PaperKiller documents I've created... why not this one?

As said, this is a limitation of the HTML Help Workshop. The TOC works within PaperKiller.
It has a bug that is activated when a chapter has both pages and subchapters.
If you make sure that pages are included only within subchapters (if a chapter has subchapters it can not have pages) it will work.
For possible workarounds, please ask Microsoft, we don't know why they introduced this limit.
Before you press the "compile" button in the HTML HELP Workshop, the TOC is available as text file (just take a look to the files automatically created by PaperKiller) so maybe Microsoft has a workaround where you should manually edit the files with the Windows notepad...
